Could you please try with the current phenix release?
1.3b-rc6:
  http://www.phenix-online.org/download/
Your phenix version is really old. In the meantime the interpretation
of PDB files was revised significantly.
Note that installations of multiple phenix versions can co-exist.
Simply make sure you source the phenix_env of the version you want
to use.
